Full Job Description

We are looking for a passionate and patient-focused Staff Nurse to join our Discharge Lounge team at Wexham Park Hospital. The Discharge Lounge is essential for maintaining hospital flow by offering a safe, comfortable space for patients waiting to go home. In your role as a Staff Nurse in the Discharge Lounge, you will work closely with the Discharge Team, Site Managers, and the ward team to enable smooth discharges of care that are patient-focused.

Main Duties of the Job

• To be responsible for the assessment, planning, implementation, and evaluation of patient care with minimal supervision.

• To carry out all administrative and nursing duties as delegated by the team leader and senior sister.

• To organise and deploy staff according to their competence to meet patients’ needs.

• To implement and be responsible for maintaining high standards of patient care.

Main Responsibilities

The Discharge Lounge nurse will assist patients with:

• Check patient observations when required to ensure safety prior to discharge.
• Administer medications as prescribed and indicated.
• Support patients with the medications they are required to take home following discharge.
• Ensure discharge medications are clearly explained by a trained member of staff.
• Provide health education and guidance at the point of discharge.
• Arrange and follow up District Nurse referrals where necessary.
• Liaise closely with Site Managers, Discharge Coordinators, and Care Providers to ensure smooth discharge planning.
• Coordinate with the pharmacy to confirm medication availability and accuracy.
• Arrange and communicate with transport services as needed.
• Offer ongoing support to patients in all matters related to their discharge process.

Person Specification

Qualifications

For the essential criteria, the candidate must be a Registered General Nurse or have pending NMC Registration. For the desirable criteria, completion of a dementia course or attendance at relevant study days will be considered an added advantage.

Experience

For the essential criteria, candidates should have recent experience working within a healthcare setting, demonstrate awareness of current professional nursing issues, and show commitment to their own professional development. For the desirable criteria, experience in caring for elderly patients with dementia and previous experience managing patients with chronic medical conditions will be considered an advantage.

Skills & Knowledge

For the essential criteria, candidates must demonstrate effective written and verbal communication skills, strong interpersonal abilities, and the capacity to manage difficult or stressful situations professionally. They should be able to prioritise their own workload as well as that of others, meet deadlines, and show strong time management and delegation skills.
